Our office provides a wide array of programs and services to the residents of Dade County, Missouri. Below is an overview of those programs.

Adult & Children's Immunizations

ALL Immunizations are available by appointment only. Call 417-637-2345 to schedule.

Medicaid and Medicare insurance plans accepted.

Bike Helmets

Bike helmets are available by request as we have limited quantities. Call our office for more information, 417-637-2345.

Birth Control

Birth control may be obtained through our office if you are a patient of our Women's Family Planning Clinic. We can also remove IUDs. If you are not part of our program we have condoms available for free. Call and make an appointment to see Dr Wolf at 417-637-2345.

Birth & Death Certificates

Vital records staff can provide certified copies of Missouri Birth certificates in our office for $15.00 each with a cash, check, debit/credit card, or money order. Identification is also required. Death Certificates are available for deaths occurring from 1980 to the present. They are $14.00 for the first copy and $11.00 for additional copies at the time of purchase. Click to print Birth/Death Certificate forms.

Car Seats

Car seats are available to families in need at the Dade County Health Department. (Limited quantities.)


  • Family must live in Dade County
  • Family must meet the WIC income guidelines.
  • Limit to one car seat per family.
  • A donation will be requested towards the replacement cost.
  • Call 417-637-2345 to make an appointment.

Environmental Services

Services include: School, restaurant, and grocery store inspections; communicable disease & foodborne illness investigations; sewage permit inspections; rabies investigations; lodging inspections; response to sanitation complaints; collection of water samples; and recall investigations. Free Clinic

We offer a Free Medical Clinic for anyone without health insurance or who has a high deductible. The Free Clinic is held on 4th Thursday of every month from 5-7pm. Call to make an appointment  417-637-2345.

Health Educator

Health Education Programs are available upon request for businesses, community groups, employee wellness plans, schools, churches, and other organizations in Dade County. Call to set up an appointment today!

IndentoGO

IdentoGO provides a wide range of identity-related services in safe, professional, and convenient locations. Our primary service is the secure capture and transmission of electronic fingerprints for employment, certification, licensing, and other verification purposes. Additional services, such as passport photos, identity history checks, and fingerprint cards are available at participating locations today and new identity-related products and services are being developed for the future. Live Well Health & Fitness Center

Our New Live Well Health and Fitness Center offers a variety of weights, machines, indoor walking track, showers,rental lockers and classes. Key Cards give members 24/7 access to facilities. The gym manager is on site Monday - Friday, 5am-7:30pm and Saturday 8:30am-2:30pm. Learn more about the fitness center here!

Low-Cost Lab Testing

Low-Cost Lab work on Monday-Thursday 8-12, must have doctor's orders. Blood pressure checks are performed daily during business hours. Free STD testing and treatment.

Pregnancy Testing

We offer pregnancy testing to the women of Dade County. This can be used to obtain Medicaid. Fee Applies.

WIC Program / Nutrition Education

Women, Infants, and Children (WIC) is a special supplemental nutrition program with a strong emphasis on education about nutrition-based health. The program provides nutrition education, screening, risk assessment, and counseling to pregnant women, new mothers, infants, and children up to five years old. We also have a Breastfeeding Peer Counselor available on WIC days and you can also call her for all for breastfeeding needs.


WIC also provides checks to purchase nutritious supplemental foods like milk, eggs, cheese, juice, cereal, peanut butter, and/or dry beans or peas at several local businesses. New mothers who are breastfeeding may receive an enhanced food package.  Learn more about WIC...

Women's Clinic

Join the thousands of Missouri women who feel good about themselves and get regular breast and cervical cancer screenings, FREE, through Missouri's Show Me Healthy Women program, click here to see if you qualify. We also have a Wise Woman program for anyone 40-64 who qualify. Click here to view Wise Woman guidelines. We offer Birth control, removal of IUDs, women's health issues, STD screening, or any other women's health issue you have concerns about. Call to make an appointment. Medicaid Women's Health accepted and a sliding scale fee available.
